For Borrower

• COE or Panunumpa sa Barangay Official
• Two(2) Government Valid Ids (photocopy)
• 2pcs of 1×1 inch or 2×2 inches photo
• Latest Payslip or Payroll Sheet
• Co-Maker (Who is also a member of the group)
• To Open Savings Account

1. P100 Open and Maintaining Account for Net Proceeds
2. P500 Open and Maintaining Account for Monthly Amortization

 For Signatories of Memoradum of Agreement for Salary Loan Assistance

    SCHOOL

• COE of Authorized signatories
• Two(2) Valid ID of signatories (photocopy)
• Payroll
• Information on date of preparation and actual date of payroll
• Secretary’s Certificate of the Authorized Signatory
• Financial Statements of the School

    BARANGAY

• Panunumpa as Barangay Official
• Two(2) Valid IDs of signatory (photocopy)
• Payroll

    SSS

• SSS pensioner with age up to 69 years old and not physically incapacitated may avail of loan with maximum amount to 1 and 1 ½ year or up to 10 Times monthly SSS pension
• Term – 1 year
• Open regular savings account with minimum initial deposit of P100.00

Requirements*

• 2 Valid Government Issued ID (SSS ID and other Government Issued ID)
• Annual Confirmation of Pensioners
• SSS benefit disbursement should be under Bangko Mabuhay

For Co-Borrower (shall be spouse or his/her dependent of legal age)

• Valid ID with three(3) Specified Signatures (photocopy)
• 1pc of 1×1 inch ID picture
